Agreement

I hereby accept and abide by the terms of Connect Performance and Connect Academy, herein referred to as CP. I understand this agreement is for the entire duration of my time at the training facility.

Indemnity

I agree to release and hold harmless CP and its employees from any loss, liability, claim of bodily harm, property damage or costs which may arise due to the use of CP equipment, facilities and my participation in CP programs, sessions, PT and activities.

Terms

Use of the facilities

The facility shall be used for training purposes only. No other activities will be allowed and violations will be sanctioned. Conducting business or paid services is not allowed.

Code of conduct

The client shall respect the behavioural and operating principles and follow them on all occasions.

Bosmansdam High School premises

The client shall respect any requirements, requests or expectations made by Bosmansdam High School, as primary operators of the site shared with CP, including security, noise, child welfare and respect for local residents.

Loss of property or theft

CP does not take responsibility for loss or theft of any personal belongings, even if locked in a locker.

Health

The client confirms that he/she does not suffer from any health problems that would hinder exercise, or that would be detrimental or adverse to the client’s health, safety or physical condition if he/she did exercise. In case of any doubt, the client shall seek advice from a medical doctor who should confirm the ability to exercise. CP shall have no obligation to perform a fitness assessment or similar testing to determine the client’s physical condition.

Insurance

The client confirms that he/she holds the necessary insurances to cover any accident or training incident. CP shall not be subject to any claim, demand or injury whatsoever with regard to the assessment of the member’s health condition or for any injury arising out of client’s disability, impairment or death.

Staff

Connect Performance staff are qualified and/or trained on an ongoing basis to deliver coaching services to adults and children, including use of all equipment held within the training facility.

Liability

Each client of CP shall be liable for any property damage and/or personal injury caused by the client at the CP premises. It shall be the obligation of the client to pay for any costs involved upon presentation of a statement thereof.

Waiver of Liability

Any and all use of CP and its facility shall be at the client’s own risk at all times. The client shall not utilise the equipment unless he/she knows the correct usage thereof. CP shall not be liable for any injuries, physical impact or damages to the client, or the property of the client, or be subject to any claim arising out of the use of the premises.
